Aug 6, 2019 · My 2016 Honda Accord had this blinking D3/03 too. This came up along with check engine light, power steering, traction control, and tire pressure lights. Car went into limp mode and lost power. Pepboys replaced my throttle body and gas pedal. Power came back and car is driving like normal. so far the only problem I have is the blinking D3/03.

I stopped by a local AutoZone and they were able to pull the code (P1740) for me and it was for a failed 4th gear switch. Not sure why your OBD reader is not picking it up.
