Internal Medicine residency isn't the hardest but also isn't the easiest. Life as an attending can be ok but I really feel like it's worth pursuing a fellowship for multiple reasons: You won't get dumped on. You can (potentially) make more money. You are less replaceable. A lot of hospitalists could be replaced with mid levels that pan-consult. It’s a bad start to your professional career if the first thing you do is lie and tell multiple programs that they are your #1 choice. Be honest. Talk about the specific reasons/experiences why you liked the program. Be genuine and sincere. If you can’t write a sincere, genuine sounding LOI, don’t write it. MOD.
